Telemedicine is now a significant element of modern healthcare provision, allowing for clinician-patient interaction at a distance and facilitating remote consultation, monitoring, diagnosis and delivery of care. But the rise of digitally-mediated healthcare has also led to a greater amount, sensitivity, and flow of health information through interdependent platforms. This poses a critical governance dilemma because patient information needs to be made available to those with permission to use it to enable timely clinical decisions, but without it being available to those not permitted to use it. Thus, addressing cyber security in telemedicine is not only a matter of technology, but also one of governance, organisation and trust. This paper explores the impact that access controls, data-governance processes and cyber security practices have on staff trust and the ability to reliably utilize telemedicine systems. It is the integration of technical security measures with explicit responsibilities, access rights, data quality controls, staff education and organisational accountability that is so important for effective security, it argues. Too tight of a control can be a hindrance to legitimate clinical activity, and weak controls can leak sensitive health information and can lead to reduced trust in digital systems. Thus, the question is how to develop a security governance model that enhances, rather than detracts from, the usability and reliability of telemedicine systems. The analysis also suggests that trust can be viewed as an effect of good governance, rather than an independent aspect of behaviour. Staff members are more prone to use digital systems if they think that information is accurate, properly protected, and accessible when needed and subject to clear institutional procedures. Therefore, the following elements should be integrated as part of a telemedicine organisation: role-based access controls, authentication, monitoring, data-governance policies, cyber security training, incident-response policies, and continuous system enhancements. A unified approach can help bolster information security and effective digital healthcare provision.
