Diabetes mellitus is a major global public health concern, characterized by chronic hyperglycaemia resulting from defects in insulin secretion, insulin action, or both. Effective management of diabetes requires continuous medical care, including regular monitoring and adherence to scheduled clinical visits. These visits are essential for glycaemic control, early detection of complications, treatment adjustments, and patient education. Non-compliance with scheduled clinical visits among diabetic patients contributes to poor glycaemic control and increased complications. To assess the prevalence and determinants of compliance among diabetic patients at Bungoma County Referral Hospital. A cross-sectional study was conducted among 64 diabetic patients selected using systematic random sampling. Data were collected using structured questionnaires and analysed using descriptive statistics and chi-square tests. Overall, 54.7% of participants were compliant, 26.6% partially compliant, and 18.8% non-compliant. Patient-related factors showed no statistically significant association (p=0.054), while hospital-related factors were significantly associated with compliance (p=0.029). Compliance to scheduled clinical visits remains suboptimal and is influenced by health system factors. Interventions targeting healthcare delivery and patient support are recommended.
